Azam started as Mzizima FC on 23 July 2004. After winning the Ilala District Championship in the fourth division in 2005, Mzizima moved into the regional league for Dar es Salaam. On 28 October 2005 a merger with other teams from the SSB group created Azam Sports Club. On 6 June 2006 the club turned full professional and became Azam Football Club. Promotion to the second division came for 2007, and the next promotion was officially sealed on 27 July 2008 with a 2–0 win over Majimaji FC.
