Modena formed on 5 April 1912 from a merger of Audax Modena and the Associazione Studentesca del Calcio Modena, and played in the top division in 1912–13. The 1916 Coppa Federale came during the First World War, and the first Serie A season followed in 1929–30. In 1959–60 the sponsor pulled out, and relegation to Serie C, a first, followed. After going bankrupt on 5 November 2017, Modena were re-founded under Romano Amadei for the 2018–19 Serie D, readmitted to Serie C in July 2019, and won Group B in 2021–22 under Attilio Tesser to regain a place in the second division.
